Purpose of education program: Hospice and palliative nurses will report increased confidence when discussing advance care planning, POLST completion and complex decision-maker situations.
Objectives:
- Identify three outcomes of successful advance care planning.
- Compare and contrast documents utilized during advance care planning.
- List five facts or benefits of completing a POLST.
- Discuss three different complex surrogate decision-maker situations.
Target Audience: APNs, RNs, LPNs – Interdisciplinary team members
The Hospice and Palliative Nurses Association is the provider approved by the California Board of Registered Nursing, Provider Number 7976, for continuing education contact hour
Attendance at the entire session and completions of a course evaluation are required to receive a continuing education contact hour certificate of completion.
